Care Management Outreach Collaboration (CMOC) coordinates professional services that help adults 18 and over with developmental disabilities and vulnerable, at-risk adults and seniors maintain their independence and prevent institutionalization in nursing homes. CMOC also assists family members and caregivers of developmentally disabled individuals and reaches out to isolated, hard-to-reach adults and seniors. CMOC is an inclusive, regional model serving over 41 communities in Los Angeles and Ventura Counties. The goal is to provide assisted living services and community education to socially and economically disadvantaged adults and seniors desiring continued independence at home.

Services

We assist in planning, arranging and monitoring services for:

In-home consultation Adult Day Program
Advocacy In-home services
Government benefits Social programs
Transportation and safety Legal services
Meal and nutritional programs Medical in-home assessments
Day care Information, referrals and assistance

CMOC's professionally trained, on-site staff work with over 20 agencies, including regional centers, senior service agencies, health care facilities, community service agencies and municipalities to gather referrals, provide assessment and connect individuals to the services needed to continue living at home.

Cost

Services are available at no cost to individuals 18 and over who exhibit mental or physical impairments which impact daily activities and those who live on a fixed income. However, donations are always accepted and appreciated. They are used to help subsidize services for needy clients.

In the San Fernando and Conejo Valleys…
Agoura Hills, Calabasas, Canoga Park, Hidden Hills, Pacoima, Reseda, Sunland, Thousand Oaks, Tujunga, West Hills, Westlake Village, Winnetka and Woodland Hills.

Care for Caregivers (CFC) Project

Care for Caregivers is a unique CMOC program that provides community education, outreach, support and supplemental services to senior caregivers in Thousand Oaks, Moorpark, and Simi Valley.
